We should have the NEW FLORIDA STATE CONSENT FORM in draft on our website this weekend. We should also have the DR. CANNABIS CLINIC DISCLOSURE & CONSENT FORM for smokable cannabis also.
If you are interested in the “smokable flower” please print these forms off and initial the forms WITHOUT SIGNING THEM. We will also have them in the office. Please call the office after this weekend, after reviewing the above mentioned forms, to make an appointment so we can request that this be added to your Medical Cannabis registration.This will be done at NO CHARGE.
